The facility alarm log did not list <br /> the L2 100 octane sump stp sump fuel alarm from 4-15-15. The facility designated operator also did not document <br /> this alarm nor the response to it. The monitoring system certification and secondary containment test results from <br /> 2015 were not found on site during the inspection. These records shall be maintained on site for at least three <br /> years. Monitoring records include: (1) date and time of all monitoring or sampling;(2) monitoring equipment <br /> calibration and maintenance records; (3) results of any visual observations;(4) results of sample analysis performed <br /> a lab or in the field;(5) togs of all readings of gauges or other monitoring equipment, ground water elevations, or <br /> other test results;(6) results of inventory readings and reconciliations. Immediately locate and maintain all <br /> maintenance and monitoring records for the last three years on site. During the inspection a copy of the 2015 test <br /> results were emailed to the facility operator. The facility operator printed them out and copies are now on site. <br /> This is a minor violation. <br /> 204 CCR 2712() Current monitoring plan approved by the EHD not found on site. <br /> The monitoring plan is not current and/or not approved by the EHD.The monitoring plan for the gas and diesel <br /> tanks indicates "Yes"for UDC Monitoring Stops Flow of Product at Dispenser. This should be marked as "No". <br /> The monitoring plan for the diesel tank lists MLLD Manufacturer Red Jacket MLLD Model FX IV D. Manufacturer <br /> should be listed as VMI and Model should be listed at 99LD2000. <br /> The monitoring plan for the used oil tank indicated Periodic System Testing"Yes"for Secondary Containment <br /> Testing. This should be listed as"No". <br /> The monitoring plan for the used oil tank lists Leak Sensor Model#302. This should be listed as 301. <br /> Therefore, an approved copy of the monitoring plans were not available for inspection. A copy of these plans shall <br /> be accessible on site at all times. The monitoring plan must be uploaded to the California Environmental Reporting <br /> System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD and <br /> retain an approved copy on site. <br /> This is a minor violation. <br /> 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the April 17, 2015, <br /> designated operator monthly inspection report and failed to check that they were responded to appropriately. The <br /> facility alarm log did not list the L2 100 octane sump stp sump fuel alarm from 4-15-15.The facility designated <br /> operator did not document this alarm nor the response to it. During the monthly inspection, the designated operator <br /> shall review the alarm history for the previous month, check that each alarm was documented and responded to <br /> appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the <br /> monthly report.
