The response plan is not current and/or not approved by the EHD. Section V has not been completed. The response plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a repeat violation, Class II. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. The site plan does not indicate the line leak detectors and does not indicate the annular sensor for the diesel/100 octane tanks. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System(CERS) at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a repeat violation, Class II. 115 CCR 2715(a) Failed to inform the EHD of designated operator identification or change within 30 days. New designated operators, James Williams and Derreq Knoules, were hired in November 2016 and notification was not provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. This is a minor violation. 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and list the used oil tank on the 11/30/16 designated operator monthly inspection report and failed to list two tanks and two dispensers on the 12/21/16 designated operator monthly inspection report.. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class ll.
