As the last operator of the UGT, it is your responsibility to <br /> investigate the extent of contamination documented at this site in a recent <br /> subsurface investigation. <br /> PHS-EHD is the lead agency assigned to provide oversight of the required <br /> investigation. You are required to submit workplans to define the lateral and <br /> vertical limits of the soil and groundwater contamination. After the plans are <br /> reviewed and approved, a boring permit application and $89 fee is required for <br /> every separate parcel where borings are proposed. Only a California C-57 <br /> contractor is authorized to drill the soil borings and install the groundwater <br /> monitoring wells. Soil and groundwater samples that are collected must be sent <br /> to a laboratory certified by the California Department of Health Services under a <br /> chain of custody. <br /> A PHS-EHD staff member will conduct inspections during drilling and sampling <br /> events and appointments must be arranged with at least forty-eight (48) working <br /> hours notice. The staff person assigned to your site will work with your <br /> environmental consultant in the field to determine which soil samples should be <br /> sent to the laboratory for analyses. PHS-EHD will review the report of findings <br /> generated by your consultant and notify you if further investigation is required <br /> and establish the date the next workplan will be due. Please have your <br /> consultant submit the report of findings within sixty (60) days of the conclusion of <br /> the field work. <br /> A Division of San Joaquin County Health Care Services <br />
