Before a <br /> J NFAR letter can be provided to you, the monitoring wells at the site must be properly destroyed. I <br /> Please submit a work plan to EHD to destroy the three monitoring wells. Note that monitoring wells <br /> in areas of contamination may only be destroyed by removing all of the well contents by drilling out <br /> the original borehole and filling it with grout. ' <br /> In addition to the above, please provide EHD certification in writing that all record fee title owners <br /> have been notified of your request for closure, This is in accordance with Section 25297.15 of the <br /> California Health and Safety Code. Submit a list of all record fee title owners and their mailing <br /> addresses.. EHD will send notification to each title owner of the pending closure letter. If you are the <br /> sole property owner, provide a statement to this effect to EHD. A form. is enclosed for your <br /> convenience.
