MARGARET EKHOLM <br /> PAGE 2 ' <br /> QUARTERLY STATUS REPORTS <br /> These reports are required to be submitted to PHS/EHD on a <br /> quarterly basis until the cleanup at the site is complete. These <br /> 3 reports should present, in a tabulated form, the groundwater <br /> monitoring and sampling data collected at the site to date. <br /> Groundwater gradient and flow direction information should also <br /> be included. <br /> Once the vapor extraction system is operational, all monitoring <br /> data obtained from the system should be included in a tabulated <br /> form. <br /> The report should include a brief discussion of the events or <br /> activities that took place during the reporting quarter and any <br /> anticipated for the following quarter. This would include the <br /> status of the construction/permitting of the vapor extraction <br /> system (include copies of any permits issued for the construction <br /> or operation of the system) , the operational status of the vapor <br /> extraction system, and any delays or problems that were <br /> encountered during the reporting quarter. <br /> s <br /> WORKPLAN FOR CONFIRMATION SOIL BORINGS <br /> After the soil remediation is complete, sufficient soil borings <br /> (samples) will need to be collected to verify the; effectiveness <br /> of the remediation system. This workplan should not be submitted <br /> i to PHS/EHD until sufficient data has been collected from the <br /> vapor extraction system to indicate the treatment has been <br /> successful. The location and number of soil borings/samples can <br /> be discussed with PHS%EHD prior to the submittal of the workplan <br /> to ensure a sufficient amount of data, is obtained. The signed <br /> permit application and $89. 00 fee should be included with the <br /> workplan to avoid an unnecessary delay. <br /> This confirmation soil data, along with the several quarters of <br /> groundwater data, will be evaluated to determine any further <br /> remedial needs or closure for the site. <br /> COMPREHENSIVE CLOSURE REPORT <br /> Should the confirmation soil and groundwater data indicate that <br /> the site is ready for closure, a comprehensive closure report is <br />. ; required. This report should include a summary of all activities <br /> conducted at the site, all data obtained from the site <br /> (tabulated) , vertical and lateral concentration contours of after <br /> contaminants found initially, and those remaining (if any) <br /> remediation, the mass balance calculation of the hydrocarbons <br /> treated versus those remaining, an explanation for any <br /> assumptions or inferences (in this case the soil contamination <br /> 3 <br />
