• Thumbs down: no/negative <br /> • Grip partner's wrist or both hands held around partner's waist: leave area <br /> immediately. <br /> 8.3 Injury or Exposure <br /> In the event of an injury or suspected exposure, the SSHO will contact the appropriate hospital <br /> and ambulance service if necessary, through 911 or the appropriate emergency number. As soon <br /> as possible after an injury, the SSHO will report the incident to the PHSM who shall investigate <br /> the circumstances surrounding the injury and file an Injury Incident Report. In the event of an <br /> accident or incident, the USACE PM shall be notified as soon as possible. Within two working <br /> days of any reportable accident, the SSHO will complete an accident report, included in <br /> Attachment B. This report will include recommendations on how to prevent occurrence of <br /> similar events. <br /> 8.4 Emergency Equipment and First Aid Requirements <br /> On-site emergency equipment will include items that are on-site for general safety use and items <br /> that Tetra Tech, Inc. will supply. These include: <br /> • Dry-chemical fire extinguisher; <br /> • Eyewash station or portable eyewash station as per ANSI Z-358.1; and <br /> • First aid kit. <br /> On-site subcontractors will also supply these items. <br /> Rough&Ready Island 26 Tetra Tech, Inc. <br /> Revised Draft HSP November 18, 1998 <br />
