9.0 SITE SAFETY AND HEALTH PROGRAM DOCUMENTATION <br /> Health and safety documentation for Rough and Ready Island will consist of the following: <br /> • Site Safety and Health Plan: All site work will be performed in accordance with the <br /> provisions stated in the HSP. <br /> • Site Visitors Log: The SSHO will maintain a visitor's log for the duration of the field <br /> activities. The Visitors log is located in Attachment C. <br /> • On-Site Safety and Health Officer's Daily Log: The SSHO will maintain a daily log <br /> that includes pertinent observations including direct-read instrumentation monitoring <br /> results (OVA) and changes in implementation of the HSP and their related justifications. <br /> (Attachment D) <br /> • Personnel Training Documentation: The SSHO will maintain documentation of site <br /> personnel training. Site workers must sign and date a personal acknowledgment stating <br /> that they have read and understood the HSP and attended the requisite site orientation and <br /> briefing (Attachment A). <br /> • Tailgate Safety Meeting Documentation: The SSHO will conduct the initial HSP <br /> conference that will serve as the initial tailgate safety meeting. This and any subsequent <br /> meeting will be documented in writing and signed by the attendees. (Attachment Q. A <br /> file of tailgate safety meeting forms will be kept by the SSHO.
