PHS-EHD e ppreciates your aggressive soil <br /> remediation procedures and groundwater investigation. Your efforts have <br /> resulted in minimizing the impact to the groundvvater and given PHS-EHD staff a <br /> high level of confidence that can allow this concurrence to occur. <br /> The existing monitoring wells (3) still in place around this site will need to be <br /> properly destroyed under a permit issued by P S-EHD or a monitoring schedule <br /> is to be established by your consultant and approved by PHS-EHD. <br /> Monitoring well destruction permits are issued to a California C-57 licensed <br /> contractor after PHS-EHD has approved a wor plan that describes the methods <br /> and materials that will be utilized. A boring/destruction permit application and a <br /> $60 application are to be submitted for review if your choice is to destroy the <br /> wells. <br /> After the wells are properly destroyed, as witnessed by a PHS-EHD field <br /> inspection, and PHS-EHD fees paid, a letter stating that no further investigation <br /> (NFI) is required can be issued. <br /> A Division of San Joaquin County H th Care Services <br />
