A letter dated 20 July 2010 was sent by the EHD explaining these fee increases and a copy of this <br /> letter and the associated attachment are appended for your review. Part of the new requirements is the need to <br /> submit the Master File Record (MFR)form which must be submitted with the permit application and must be <br /> signed by the person taking responsibility for any additional fees incurred. A copy of the MFR is also <br /> appended along with a copy of the new well and boring permit application. All of these forms can be downloaded <br /> from EHD's website w_ww.siggv.org/ehd <br /> I have totaled the cost of the fieldwork involving the advancement of three borings on one parcel, and five borings <br /> plus one monitoring well on another parcel as follows: <br /> 523 Larch Road <br /> All 5 borings $122 <br /> One monitoring well $122 <br /> Three-hour inspection fee - $366 <br /> 3940 N.
