Propane—the hazardous materials type, average daily amount, and federal hazard categories have not <br /> been completed. <br /> 2. Refrigerant R408A—the hazardous materials type and federal hazard categories have not been <br /> completed. <br /> 3. Refrigerant Forane R502-the hazardous materials type and federal hazard categories have not been <br /> completed. <br /> 4. REFRIGERANT R408A#2 -the average daily amount, days on site, storage pressure, storage <br /> temperature, and federal hazard categories have not been completed, and the amount of Freon must <br /> be reported in pounds. <br /> 5. Calcium chloride-the federal hazard categories have not been completed. <br /> 6. Nitrogen gas—the average daily amount, days on site, storage pressure, and storage temperature have <br /> not been completed. <br /> 7. REFRIGERANT 410A-storage pressure is wrongly identified as "ambient"vs. "above ambient" and the <br /> amount of Freon must be reported in pounds. <br /> • Violation#6,the site map does not show or address the following: <br /> 1. Access and exit points from the street. <br /> 3. Emergency response equipment type. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, <br /> and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and <br /> label as "NA". <br /> • Violation #11,the training records uploaded to CERS do not reflect relevant training(see attached sample <br /> employee training record; do not upload training records to CERS, submit training records directly to the EHD). <br /> 1 <br />
