DISCHARGE NOTIFICATION (40 CFR 112.7 (a)(4)) <br /> Any discharge that affects or threatens to affect navigable waters must be immediately reported to the <br /> National Response Center. The National Response Center is staffed 24 hours a day. The person <br /> reporting the discharge must provide the National Response Center with: <br /> • Their name, location, organization, and telephone number. <br /> • Name and address of the parry responsible for the incident. <br /> • Date and time of the incident. <br /> • Location of the incident. <br /> • Source and cause of the release or discharge. <br /> • Types of material(s)released or discharged. <br /> • Quantity of materials released or discharged. <br /> • Danger or threat posed by the release or discharge. <br /> • Number and types of injuries (if any). <br /> • Media affected or threatened by the discharge (water, land, air). <br /> • Weather conditions at the incident location. <br /> • Any other information that may help emergency personnel respond to the incident. <br /> • Corrective action and countermeasures you have taken, including a description of equipment <br /> repairs and replacements. <br /> • Additional preventive measures you have taken or contemplated to minimize the possibility of <br /> recurrence; and <br /> • Such other information as the Regional Administrator may reasonably require pertinent to the <br /> Plan or discharge. <br /> Upon receipt of the information such State agency or agencies may conduct a review and make <br /> recommendations to the Regional Administrator as to further procedures, methods, equipment, and other <br /> requirements necessary to prevent and to contain discharges from the Facility. <br /> Joe's Travel Plaza <br /> 15600 Harlan Road,Lathrop,CA <br /> Page 27 <br />
