B St. #B, Stockton <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />ROUTINE HAZARDOUS WASTE GENERATOR INSPECTION <br />Class II violations: (15, 60, 61, 62) <br />15. Two spill pallets inside the shop were observed with standing liquid in the bottom. All facility <br />communications and alarm systems, fire protection equipment, spill control equipment, and <br />decontamination equipment, shall be tested and maintained as necessary to assure its proper operation <br />in time of emergency. Spill pallets shall be maintained clean and dry at all times so as to detect a leak at <br />the earliest opportunity. Immediately remove and properly dispose of all hazardous waste from spill <br />pallets, and maintain all spill containment clean and dry at all times. <br />REPEAT VIOLATION <br />60, 61, & 62. Manifest number 002390158JJK (8/25/08) was not found on site. Manifest number <br />006661378JJK (5/23/11) was found without a signed off copy from the destination facility. Hazardous <br />waste generators shall retain copies of all manifests signed off by the destination facility on site for three <br />years and have them readily available for review. If the generator did not receive a copy of the manifest <br />with the handwritten signature of the owner or operator of the facility to which the generator's waste was <br />submitted within 35 days of the date the waste was accepted by the initial transporter, the generator <br />shall contact the transporter and/or the owner or operator of the designated facility to determine the <br />status of the waste. If the generator did not receive a copy of the manifest with the handwritten <br />signature of the owner or operator of the facility to which the generator's waste was submitted within 60 <br />days of the date the waste was accepted by the initial transporter, the generator shall submit a legible <br />copy of the missing manifest, with some indication that the generator has not received confirmation of <br />delivery. This information shall be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit the required information to <br />DTSC.
