B St. #B, Stockton Program: HW SUMMARY OF VIOLATIONS CLASS I, CLASS II, or MINOR -Notice to Comply) ROUTINE HAZARDOUS WASTE GENERATOR INSPECTION Class II violations: (15, 60, 61, 62) 15. Two spill pallets inside the shop were observed with standing liquid in the bottom. All facility communications and alarm systems, fire protection equipment, spill control equipment, and decontamination equipment, shall be tested and maintained as necessary to assure its proper operation in time of emergency. Spill pallets shall be maintained clean and dry at all times so as to detect a leak at the earliest opportunity. Immediately remove and properly dispose of all hazardous waste from spill pallets, and maintain all spill containment clean and dry at all times. REPEAT VIOLATION 60, 61, & 62. Manifest number 002390158JJK (8/25/08) was not found on site. Manifest number 006661378JJK (5/23/11) was found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. By September 12, 2011, submit to the EHD a copy of the signed off copy of the manifests or a copy of the information submitted to DTSC. REPEAT VIOLATION ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
