Charter Way, Stockton Date Oct 18, 2011 <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />ROUTINE HAZARDOUS WASTE GENERATOR INSPECTION <br />**Inspection conducted on October 18, 2011. Report completed on October 19, 2011. Checklist left on <br />site at time of inspection. <br />Class II violations: (9999) <br />9999. The hazardous training is being directed by Tom Garing, Jennifer Gutierrez, and Julio Pineda, who <br />are lacking current training. Previously training was directed by Kosal Vong, whose HAZWOPER training <br />expired in 2009. Facility personnel shall complete a training program that teaches them to perform their <br />duties in a way that ensures the facility's compliance with Title 22 regulations. This program shall be <br />directed by a person trained in hazardous waste management procedures. Immediately conduct <br />hazardous waste management training program for this facility in accordance with Title 22 regulations, <br />which is directed by persons who have current hazardous waste training, and ensure that all employees <br />complete this training within six months of hire and at least annually thereafter. Submit a copy of the <br />updated training program and records to the EHD by November 19, 2011. <br />Complete and submit a copy of the Return to Compliance Certification to the EHD by November 19, 2011 <br />along with a statement documenting the corrective actions that have been or will be taken for each <br />violation and any supporting paperwork. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE <br />VIOLATIONS <br /># <br />C1 <br />C2 <br />M <br />Code Section <br />HSC = CA Health and Safety Code; CFR = Code of Federal Regulations <br />31 = Class 1 violation.
