P.G. <br /> Project QC Manager/Correspondence & Submittals Coordinator Suman Sharma <br /> Field Manager James Medley <br /> Field Quality Control Engineer/SSHO Lik-See Chung, P.E. <br /> The Corporate QC Manager, Mr. John Fleissner, will oversee all OTIE QC activities, and will <br /> ensure that appropriate resources, training, and lines of authority are established in support of <br /> this project. <br /> The Project QC Manager, Ms. Suman Sharma, will provide overall project quality control <br /> oversight, certify that the project has been completed in accordance with the Work Plan, and <br /> coordinate correspondence and submittals on QC issues with the CA ARNG Remedial Project <br /> Manager, as appropriate. In the event that a corrective measure is required, the Project QC <br /> Manager will verify that the corrective action is approved by the Project Manager and has been <br /> implemented accordingly. <br /> The Quality Control Engineer, Mr. Lik-See Chung, is the lead on-site person responsible for <br /> determining that project field activities are being performed in compliance with the Work Plan. <br /> No work will be performed without Quality Control Engineer oversight. Mr. Chung will also serve <br /> as the Site Safety and Health Officer, and will be responsible for ensuring that the APP/SSHP is <br /> fully implemented. <br /> The Field Manager, Mr. James Medley, will be responsible for all field activities at the site, <br /> including safety, quality and production. He will maintain a physical presence at the site at all <br /> times when site work is being performed and will report directly to the Project QC Manager. The <br /> Construction Manager has the authority to stop work for safety violations and remove personnel <br /> that fail to comply with the safety program (including subcontractors). <br /> To ensure that the project complies with the Work Plan and applicable contract requirements, <br /> the project team will implement a three-phase QC system, as follows: <br /> • Preparatory Phase: The Work Plan and APP/SSHP are thoroughly reviewed, all <br /> required ensure that all preliminary work is complete and in compliance with the <br /> contract, and that materials are mobilized to the site and certified to be present, the work <br /> area is examined to any subcontractors performing work have the appropriate insurance <br /> documentation; <br /> • Initial Phase: Implementation of controls developed for the project during the <br /> Preparatory Phase and performance of the work to the required level of workmanship <br /> are verified; and <br /> Oneida Total Integrated Enterprises, LLC(OTIE) Page 4 of 5 <br />
