APP <br /> Well Installation and Sampling at California National Guard Field Maintenance Shop <br /> Stockton, California <br /> 2.2.2 Description of California National Guard Armory in Lodi, CA <br /> AV <br /> 1 <br /> ;M <br /> r <br /> 1 <br /> i <br /> The California National Guard Armory is located at 333 North Washington Street in Lodi, CA <br /> (See Figure 1). The Lodi Armory facility is located between East Lockeford Street to the south, <br /> East Lawrence Avenue to the North and North Stockton Street to the West. <br /> The Armory facility serves as a troop training and assembly area. Additionally, the <br /> Organizational Maintenance Shop (OMS) provides vehicle maintenance and service including <br /> tune ups, oil changes, minor maintenance, painting and washing. <br /> The potential contaminant found in the soil is Total Petroleum Hydrocarbon as Diesel. <br /> Figure 2 shows the project monitoring well locations at the California National Guard Armory in <br /> Lodi, CA. <br /> 2.3 CONTRACTOR ACCIDENT EXPERIENCE <br /> The OTIE Environmental Health and Safety (EHS) Performance Summary, including <br /> Experience Modification Rate (EMR); injury and illness incident rates; Days Away, Restricted, <br /> Transfer (DART) incident rates; etc., is included in Appendix A of this APP/SSHP. Also included <br /> is the OTIE 2008 Occupational Safety and Health Administration (OSHA) Form 300A Summary <br /> of Work-Related Injuries and Illnesses, and the OTIE corporate trend analyses. <br /> 2.4 LISTING OF PHASES OF WORK AND HAZARDOUS ACTIVITIES REQUIRING ACTIVITY <br /> HAZARD ANALYSES <br /> It is recognized that certain work activities are inherently more hazardous than others. For these <br /> operations, a detailed activity hazard analysis (AHA) will be conducted for each associated work <br /> task that specifies corrective actions to be taken to control the hazard. AHAs will be produced, <br /> reviewed, and modified, as necessary, to address new work activities and changing site <br /> conditions as the work progresses. Work will not begin until the AHA for a given work activity <br /> has been accepted by the client and discussed with all affected project personnel. The <br /> OTIE 2 <br />
