APP <br /> Well Installation and Sampling at California National Guard Field Maintenance Shop <br /> Stockton, California <br /> medical facility, he/she must report to their local business office prior to returning to work. Under <br /> no circumstances are employees with medical restrictions to report back to the field site prior to <br /> being released from the business office. All work restrictions will be discussed with the <br /> employee's supervisor and his/her doctor. If there are lost work days because of a Workers' <br /> Compensation case, the employee must obtain a "work release" from the doctor and a "return to <br /> work" slip and report to the business office. If there are any changes in the medical status of a <br /> Workers' Compensation claim, the employee should report the change immediately to the <br /> business office. <br /> 8.1 RESPONSIBILITIES <br /> In order to accomplish the objectives of this program, a team effort on the part of all OTIE <br /> employees, including subcontractors, is needed. It is essential that all personnel take an interest <br /> and participate actively in all phases of the Accident and Crisis Management Program. <br /> OTIE 19 <br />
