APP <br /> Well Installation and Sampling at California National Guard Field Maintenance Shop <br /> Stockton, California <br /> 9.2.5 Man Overboard/Abandon Ship <br /> This section is N/A to the California National Guard Armory site in Lodi, CA. <br /> 9.2.6 Medical Support <br /> The purpose of the OTIE medical surveillance program is to provide uniform medical care, <br /> ensure the selection of employees physically able to safely perform the work assigned, monitor <br /> employee health on a regular basis, and provide medical care for occupational injury or illness. <br /> This program applies to all OTIE employees and subcontractors. <br /> In the event of serious personnel injury (fatality, patient unconscious, possibility of broken <br /> bones, severe bleeding, burns, blood loss, shock, or trauma), the first responder shall <br /> immediately: <br /> • Administer first aid if qualified; if not qualified, immediately seek out a person qualified to <br /> administer first aid: NOTE: the SSHO will be a current First Aid and CPR Responder; <br /> and <br /> • Notify the Site Emergency Coordinator(in this case, the SSHO) of the name of the <br /> victim, their location, and the nature of injury. <br /> The SSHO or designee upon receipt of notification of the injury, shall immediately: <br /> • Notify emergency response and give the appropriate patient information and their <br /> location. <br /> • Assist the injured party as deemed appropriate. <br /> • Provide a copy of the injured party's medical data sheet to responding medical <br /> personnel. <br /> • Designate someone to accompany the injured party to the hospital and to provide <br /> chemical data sheets to the emergency medical team. <br /> • Notify the PM, SHM, and COR if not already notified. <br /> • Complete an OTIE Incident Report and Investigation Form. <br /> If the SSHO determines that emergency response is not necessary (minor injury such as sprain <br /> or abrasion, patient is conscious and can be moved), he/she may direct someone to transport <br /> the patient by vehicle to the hospital. A hospital route map will be located in all site vehicles and <br /> is included as Figure 4. The SSHO will then file an Accident Report with the SHM. <br /> In the event of an on-site injury at the California National Guard Armory requiring emergency <br /> medical care or emergency response, the following medical center will be used: <br /> Lodi Memorial Hospital <br /> 975 S. Fairmont Avenue <br /> Lodi, CA 95240 <br /> Phone: 209-334-3411 <br /> OTIE will be utilizing the Resident Physician(s) and/or On-Call Staff at Lodi Memorial Hospital <br /> for emergency situations. <br /> OTIE 24 <br />
