APP/SSHP <br /> Soil Excavation at California National Guard Armory <br /> Lodi, California <br /> 2.0 KEY PERSONNEL AND RESPONSIBILITIES <br /> This section describes the personnel responsibilities for implementation of the SSHP. Clear <br /> lines of authority will be established for enforcing compliance with the SSHP. More information <br /> is given in the APP, in Section 4. <br /> Designated OTIE personnel are responsible for field implementation of the SSHP. This includes <br /> field supervision, enforcing safe work practices, ensuring proper use of PPE, and <br /> communicating modified safety requirements to site personnel. <br /> To meet the OTIE goal of ZERO INCIDENTS and SAFETY EXCELLENCE and to meet its <br /> project safety and health objectives, the Project Team has developed a line of reporting and has <br /> tasked individuals with the safety and health responsibilities outlined in this section. <br /> 2.1 PROJECT MANAGER (PM) <br /> Michael Quillin: Acquaint field personnel with potential hazards and procedures to minimize the <br /> negative impact of those hazards. Make available proper PPE and other safety equipment, <br /> adequate time and budget, and trained personnel to perform site work in a safe manner. <br /> Arrange for preparation of a SSHP. <br /> 2.2 SAFETY& HEALTH MANAGER(SHM) <br /> William S. Fink, CIH, CSP, CHMM: Be responsible for the development, implementation, <br /> oversight, and enforcement of the SSHP. Sign and date the SSHP prior to submittal. Be <br /> available for emergency consultation. Provide consultation as needed to ensure the SSHP is <br /> fully implemented. Coordinate any modifications to the SSHP with the PM, the FTL, the SSHO <br /> and the Contracting Officer (CO). Review and approve SSHP amendments for specific site <br /> tasks. Review accident reports and results of daily inspections. Serve as a member of the <br /> Contractor's quality control staff. Coordinate with the FTL and SSHO regarding corporate policy <br /> or procedure changes that may impact the project. <br /> 2.3 FIELD TEAM LEADER (FTL) <br /> James Medley: The FTL shall ensure that site personnel have read, signed, and will follow the <br /> master copy of this document. Investigate and report to the FTL each work-related illness or <br /> injury, near misses, accidents, and damage to physical property. <br /> The 10-hour OSHA Construction Safety training (minimum) and current cardiopulmonary <br /> resuscitation (CPR)/First-Aid certification is required for the FTL. Evidence of current training <br /> must be readily available. <br /> 2.4 SITE SAFETY& HEALTH OFFICER (SSHO) <br /> Lik-See Chung: Ensure that site personnel have read, signed, and will follow the master copy of <br /> this document. Coordinate with the SHM on accident investigations, as necessary. Assist and <br /> represent the OTIE SHM in on-site training and the day-to-day onsite implementation and <br /> enforcement of the accepted SSHP. Be assigned to the site on a full-time basis for the duration <br /> of field activities. Conduct initial site-specific training. Have authority to ensure site compliance <br /> with the following: <br /> • Specified safety and health requirement including Federal, State, and OSHA regulations. <br /> • Enforcement of all aspects of the SSHP including, but not limited to: <br /> OTI E 3 <br />
