The report was evaluated in accordance with <br /> the Local Agency Groundwater Protection/Underground Injection Control Program guidelines prepared by <br /> the United States Environmental Protection Agency [USEPA - Region 9 "Guidelines for Closure of <br /> Shallow Disposal Wells"(1992)7. <br /> PHS/EHD witnessed the destruction and sampling of the four disposal wells in the storm water pond at the <br /> site referenced above between July 17-20,2000. <br /> This letter confirms the completion of a Local Agency Groundwater Protection/Underground Injection <br /> Control Program (UICP) site investigation and disposal well closures (4) formerly located at the above- <br /> described location. Thank you for your cooperation throughout this investigation.
