The report describes the potential improper use of four shallow
injection/disposal wells constructed inside a storm water evaporation pond on-site.
PHS/EHD has recently implemented a Local Agency Groundwater Protection/Underground Injection
Control Program in accordance with guidelines prepared by the United States Environmental Protection
Agency(USEPA). The primary focus of the program is the identification, evaluation, and potential closure
of improper disposal wells and identification of other land use practices in San Joaquin County that may
lead to degradation of drinking water supplies. PHS/EHD seeks your cooperation in providing information
that will assist in the evaluation of your facility.
As the responsible party PHS/EHD requests that you describe any on-site liquid/fluid disposal. This
includes, but is not limited to, construction details of the on-site disposal wells, septic, storm water, and
wastewater conveyance systems. Please include copies of any construction permits or waste discharge
permits. Please describe in detail the past and present water uses, fluid and sludge waste stream
characterization (analyses), and wastewater disposal methods at this facility including any pre-treatment,
recycle,storage,on-site and off-site disposal with supporting documentation if any.
This correspondence is issued to the owners of record for the property described above. PHS/EHD requests
the property owner read, complete, sign, and return the enclosed Master File Record Form (MFR) and
submit the required 3 hour minimum advanced fee for services totaling $234.00(Payment to: PHS/EHD).
This site is currently classified as an Underground Injection Control Program Environmental Assessment,
assigned to the Site Mitigation Unit at PHS/EHD.
Federal, State, and local regulations control injection/disposal well construction, use, or installation.
Following a review of the information submitted, PHS/EHD has determined that the disposal wells at the
above referenced site were installed and operated without approval from any authorized permitting agency.
Laboratory analyses of soil/sludge from the on-site disposal wells dated May 23,2000 indicate the presence
of contaminants potentially injected into the subsurface via the on-site disposal wells.
A Division of San Joaquin County Health Care Services
