A report titled 3r`t Quarter Groundwater Monitoring dated November 8, 1999 by <br /> Geological Technics Inc. (GT) for the above referenced site suggests that the lateral and vertical extent of <br /> contaminant distribution is currently undefined, particularly downgradient. Groundwater gradient and flow <br /> direction trends toward the on-site domestic drinking water well. In correspondence from PHS/EHD dated <br /> August 6, 1999 a request for information including "a copy of the calculations used to determine the <br /> groundwater gradient" at this site has not been received. For all future quarterly groundwater monitoring <br /> reports please include all data, formulas and calculations for groundwater gradient determinations for <br /> verification purposes. Groundwater plumes which may migrate toward drinking water supply wells are <br /> considered high risk scenarios where mitigation measures should be implemented promptly. Please <br /> continue quarterly groundwater monitoring, sampling,analyses and reporting at this site. <br /> On August 6, 1999 PHSIEHD also directed the responsible party to "submit a workplan to PHSIEHD <br /> which when implemented will define the soil and groundwater plume at this site". On September 17, 1999 <br /> PHS/EHD correspondence granted an extension for the submittal of the workplan to October 15, 1999. As <br /> of this date PHS/EHD has not received the required workplan or gradient calculation information. The <br /> responsible party at this site is no longer in compliance with correction action directives. Failure to comply <br /> may affect your reimbursement eligibility(Underground Storage Tank Cleanup Fund). <br /> You are now directed to submit a workplan to PHS/EHD which when implemented will define the <br /> soil and groundwater plume at this site.
