Page 4 of 27 <br /> sampling shall be reported in the month following analysis and shall be included <br /> in the self-monitoring report, or if no self-monitoring report is due in the <br /> following month the analysis shall be reported to the City by the 20`}' of the <br /> month. Increased monitoring results shall be identified when included in a <br /> required self-monitoring report. <br /> C. Automatic Resampling <br /> If the results of the permittee's wastewater analysis indicates that violation of this <br /> permit has occurred, the pennittee must: <br /> 1. Inform the City of the violation within 24 hours; and <br /> 2. Repeat the sampling and pollutant analysis and submit, in writing, the <br /> results of this second analysis within 30 days after the first violation <br /> became known. <br /> D. Accidental Discharge Report <br /> 1. The permittee shall notify the City immediately upon occurrence of an <br /> accidental discharge of-substances prohibited by the Waste Discharge <br /> Ordinance or any slug loads or spills that may enter the public sewer. The <br /> notification shall be given to the Water Quality Control Facility at 209= <br /> 239-8433. The notification shall included: <br /> a: The facility y name and location of the dischar e. <br /> b. The date, and time of the discharge. <br /> c.. The type of waste, including the concentration and volume. <br /> d. The corrective actions taken. <br /> 2. Within five days following an accidental discharge, the permittee shall <br /> submit to the City a detailed written report. The report shall specify: <br /> a. Description and cause of the upset, slug load, or accidental <br /> discharge, the cause thereof, and impact on the permittee's . <br /> compliance status. The descriptionshould also include location <br /> of discharge, and the type, concentration and volume of waste. <br /> b. Duration of noncompliance, including exact dates and times of <br /> noncompliance and, if the noncompliance is continuing, the date <br /> and time by which compliance is reasonably expected to occur. <br /> c. A14 steps taken or to be taken to reduce, eliminate, and/or prevent <br /> recurrence of such upset, slug load, accidental discharge, or other <br /> condition of noncompliance. <br /> E. All reports required by this permit shall be submitted to the City at the following <br /> address: <br /> Permit No. 05-3 <br /> Cardoza Enterprises <br />
