Diamond Lumber-Lodi,CA Page 2 <br /> The first sentence of the fifth paragraph states a PHS/EHD staff member noted during the <br /> monitoring well sampling on November 25, 1991,the blue ice in the cooler was not frozen,.and <br /> not capable of maintaining the samples in cool ,condition. 'It should be noted that Margaret <br /> Lagorio (PHS/EHD) was on-site during monitoring well sampling, and mentioned to the SRK <br /> field hydrogeologist that the water samples was not sufficiently cool. SRK complied with her <br /> request and additional frozen blue ice was placed in the cooler prior to shipment to the <br /> laboratory: The Chain-of-Custody states the water samples were received in good condition and <br /> cool (40C). <br /> The second sentence of the fifth paragraph states informnation regarding transport of the water <br /> samples'tfrom California to Washington is not'documented on the Chain-of-Custody. The Chain- <br /> of-Custody includes the sample location in the Project Name, 'Diamond Lumber-Lodi', and in . <br /> the Sample Identification, "SRK-MWl-LODP'. The top of,,the-Chain-of-Custody form clearly <br /> states the name and address of North Creek Analytical in Bothell, Washington. Please note that <br /> the copy of .the Chain-of-Custody was relinquished on November 26, 1991, by SRK. <br /> Unfortunately the copy of the Chain-of-Custody that was included in the report was photocopied <br /> from a very light carbon copy that did not reproduce the signature, date, and'time the samples <br /> were relinquished.
