LOP SITE CODE: 1 356 APN: 02704004 CVRWQCB CASE # 390096 E 03/05/87 Due to an environmental assessment request, four soil borings were drilled, two borings were grouted (SA, SB) and the other two were made into monitoring wells (S-1, S-2) on this date. Soil and groundwater contamination were reported. 03/16/87 A "Preliminary Site Assessment Report" was submitted to the Environmental l Health Department (EHD) on this date that detailed the work performed in March 1987. 08/12/87 On this date three more monitoring wells (MWI through MW3) were installed. 10/13/87 A "Delineation Report" was submitted to the EHD on this date that detailed the August 1987 work. 01/20/89 On this date monitoring wells MW4 through MW7 were installed. 05/13/91 EHD placed the site in Local Oversight Program and sent a Notice of Responsibility to USA'Petroleum, the responsible party. ' 02/08/93 On this date monitoring well MW8 was installed. 03/14/95 On this date a soil vapor extraction test was performed on monitoring wells S-1, S-2, MW3, and MW8. - r 04/17/95 The date of "Results;of Pilot Test for Vapor Extraction" that summarizes the vapor extraction test performed in March 1995. 1016-7197 Three gasoline underground storage tanks and piping were removed from the site, excavation and disposal of 500 cubic yards of contaminated soil from the UST area occurred and sampling was performed beneath the piping and in the excavation. 10/22/97 The date of the "Soil Excavation and Product Line Sampling" report discussing the October 1997 work. 01/26/99 The date of the-"Remedial Action Plan" that proposed remediation of the site. 05/4-5/00 On this date air sparge well ASI, monitoring wells MW9, and MW10 were installed and MW5 was destroyed. A report of this work was prepared in May 2000. 08/10/00 A public notice was posted at the site on this date for the planned remediation of soil vapor extraction and air sparging (SVE/AS). i y x
