At the time of inspecti <br />n, it could not be demonstrated (not necessarily documented) that <br />employees who handle hazardous <br />waste were properly trained. According to Mr. Butch Hobson, the <br />teachers have not been given <br />annual hazardous waste training, however, a training class has been <br />scheduled for tomorrow. The <br />generator must ensure that all employees who handle hazardous waste <br />are thoroughly familiar with <br />proper waste handling and emergency procedures. Immediately provide <br />training to all employees who <br />handle hazardous waste and submit a copy of the training records to the <br />EHD by November 17, 2011. <br />812. Copies of universal waste <br />disposal records were not available on site. A generator of universal <br />waste (batteries, mercury containing <br />equipment, lamps/bulbs, electronic devices, CRTs/CRT glass, <br />aerosol cans) shall keep a record <br />of each shipment of universal waste on site for at least three years. A <br />blank log was observed on <br />site for future disposal. The record may take the form of a log, invoice, <br />manifest, bill of lading, oro <br />her shipping document, and shall include the following: <br />- Name & address of destination <br />facility <br />- Quantity & type of universal <br />waste <br />- Date of departure/shipinent <br />Begin this practice immediately. <br />Complete and submit a copy <br />of the Return to Compliance Certification to the EHD by November 17, <br />2011 along with a stateme <br />t documenting the corrective actions that have been or will be taken for each <br />violation and any supporting <br />paperwork. <br />**Facility generated 0.475 t <br />ns of hazardous waste in 2010. <br />Note: (corrected on site) EI <br />ctronic waste was observed labeled only with the accumulation start date. A <br />generator shall label or mak <br />universal waste with the words 'Universal Waste' and the type of waste <br />(alkaline batteries, mercury <br />containing equipment, fluorescent bulbs, electronic devices, CRTs, aerosol <br />cans). The labels were cor <br />ected at the time of inspection. <br />Note: A hazardous waste <br />manifest for methyl ethyl ketone disposal was available for 4/12/10. Numerous <br />chemicals and animal specimens <br />were disposed of on hazardous waste manifests on 10/1011.
