Summary of Well Destruction and Request For No Further Action Letter 2 <br /> Fuller Mobtle Home Park 3M5 East Louise Avenue Lathrop,CA October 21, 1998 <br /> V&W Drilling The destruction of the seven wells was observed and approved by Ms Margaret <br /> Lagorlo with the SJCEHD <br /> Well MW-4, located in the City right-of-way, was pressure grouted to three feet below grade surface <br /> (bgs) using neat cement The well box and 3 feet of casing was removed, the void filled with a 2 <br /> sack sand/cement slurry, and the remaining 4 inches was capped with concrete and finished at grade <br /> to match the sidewalk <br /> Wells MW-5, MW-6, and MW-7 were destroyed by pressure grouting Each well was filled with <br /> neat cement to three feet bgs, the well box and upper three feet of casing was removed, and the <br /> remaining void filled with native soil to match existing grade <br /> Since field conditions would not allow the drill rig to access well number MW-3, Ms Lagorio <br /> approved destruction of well MW-3 via pressure grouting Well MW-3 was destroyed by pressure <br /> grouting using neat cement to within three feet bgs The well box and upper five feet of casing was <br /> removed and the remaining void filled with an additional two feet of grout and capped with native <br /> • soil to match existing grade <br /> Wells MW-I and MW-2 were overdrllled to 25 feet using a truck-mounted drill rig equipped with <br /> 8-inch diameter hollow stem augers Each well was tremae grouted to 3 feet bgs through the augers <br /> using neat cement, the well boxes and upper 3 feet of casing were removed, and the void was filled <br /> with native soil to match existing grade The drill cuttings generated during the overdnlling was <br /> placed 1n 55-gallon drums The augers were steam cleaned between each well and the decon water <br /> was placed in a 55-gallon drum Each drum was labeled and stored on site A total of 4 drums of <br /> drill cuttings and I drum of decon water were generated during the well destruction activities <br /> Waste Profiling and Disposal <br /> All PVC casing, concrete, and well boxes generated during the well destruction work were removed <br /> by the drilling company for disposal at a local landfill <br /> One discrete soil sample was collected from each of the drums and labeled SS-I through SS-4 The <br /> samples were collected In a 6-inch-long by 2-inch-diameter brass cylindrical sleeves that were sealed <br /> with teflon pads and plastic caps, labeled, and placed in a cooler on ice Two 40 ml VOA's were <br /> . filled with decon water stored 1n the 55-gallon drum The water samples were labeled as Decon-1 <br /> File ProaectlFullerMobile Ho1ne1WellDe$tructl LTR <br />
