Louise Ave.
Manteca CA 95336
i
i
San Joaquin County Public Health Services,Environmental Health Division (PHS/EHD) has reviewed the
"Work Plan for Monitoring Well Installation"that was submitted by Compliance&Closure, Inc. (CCI)for
the above referenced site and has the following comments for your consideration.
The work plan proposes the installation of three monitoring wells on site to establish gradient and monitor
groundwater quality over time. The proposed location of the wells was as follows: One well near the
north side of the former tank pit area, one well east of the new tank farm and one well south west of the
new tank farm. In a conversation on March 8, 1999 between CCI and PHS/EHD it was agreed that the
well proposed for the south west corner of the site be moved to the north east corner, closer toarea of
established ground water contamination.
During the initial site investigation, which took place on October 19-20, 1998, groundwater was
encountered at approximately 15 feet below surface grade(bsg). The proposed wells will likely have a
screen interval beginning at ten feet bsg and.extending to 25 feet bsg.
The work plan is approved with the above noted and agreed upon change in well location. Please submit
to PHS/EHD a completed boring permit application and required $89.00 fee. Please note that 48 hours
notice to PHS/EHD is required prior to scheduling fieldwork. j
1
If you have any questions or comments or wish to schedule fieldwork please call Lori Duncan at(209) j
468-0337.
i
i
Donna Heran, REHS, Director
Environmental Health-Division
Lori Duncan, REHS MargareYLagorio, REH
Site Mitigation Unit IV Supervisor, Unit IV
I
cc: Mark List, CVRWQCS
George Yamashita, c/o.Jeffrey Widman
Gary Mulkey, CCI
A,Division of San Joaquin County Health Care Services
