(AGE), on your behalf for the above- <br /> referenced site. <br /> AGE proposes to destroy monitoring wells (MW) MW-1 through MW-8 and MW-11, soil vapor <br /> extraction wells (VW) VW-1 through VW-3, and ozone sparging wells (OZ) OZ-1 and OZ-2 by <br /> over-drilling the wells one foot deeper than their installed total depths. Resulting boreholes are <br /> proposed to be backfilled to surface grade with neat portland cement and covered with material <br /> to match surrounding surface. <br /> AGE proposes to destroy MW-9 and MW-10 by pressure-grouting. Wells are proposed to be <br /> filled over their entire depths with a grout mixture, 25 pounds per square inch (psi) pressure <br /> applied for a minimum of 10 minutes, and a grout "mushroom cap" installed to near surface <br /> grade. <br /> The EHD approves the well destruction methods proposed in the Work Plan with the following <br /> conditions: <br /> • Mix neat cement (grout) at a ratio of one 94-pound sack of portland cement with 5 to 6 <br /> gallons of'clean' potable water; <br /> • Remove well boxes prior to destroying the wells; <br /> • Over-drill MW-11 using an 8-inch auger; <br /> • Regarding over-drilled wells, fill boreholes with grout, pumped from the bottom of the <br /> borehole up to at least 3 feet bsg through a tremie pipe that extends to within 5 feet of <br /> the borehole's total depth; <br /> • Regarding wells proposed to be destroyed by pressure grouting, enlarge the diameter of <br /> the top 3 feet of the borehole at least one foot larger than the auger originally utilized to <br /> install the well; after the grout mixture has completely filled the casing, apply at least 25 <br />
