Well Destruction Work Plan - Response Page 2 of 3 <br /> California Stop February 23, 2015 <br /> 2224 Manthey Road <br /> Stockton, California 95206 <br /> psi pressure on the grout for ten minutes (destroy the well by over-drilling if the well <br /> casing is unable to withstand this pressure for this length of time); cut off and remove the <br /> casing 6 inches above the bottom of the excavation before inserting grout into the <br /> excavation to create a "cap" with a vertical thickness of one Foot; this "cap" must be <br /> installed in one continuous pour immediately after the well has been destroyed; <br /> • All Investigation Derived Wastes (IDW), including all soil cuttings, debris, and <br /> groundwater generated from well destruction activities, are required to be properly <br /> containerized, labeled, stored and disposed; <br /> • A well destruction report describing all well destruction field activities and including the <br /> receiving facility-signed manifests for the disposed IDW must be submitted to EHD; <br /> To proceed with obtaining the destruction permit for these wells, the following must be <br /> submitted to the attention of Gabriel Gonzalez at the EHD: <br /> • EHD Well and Boring Permit Application form; <br /> • Master File Record form; <br /> • Licensed Contractor's Declaration (LCD); and <br /> • A fee of $2,470 (16 x $130 + 1 x $390). The permit fee for the destruction of each well is <br /> $130, along with a $390 Inspection Activity fee. <br /> After the EHD permit has been issued, the EHD requires that inspections be scheduled with <br /> Johnny Yoakum, REHS at least 5 working days prior to the commencement of field work; please <br /> contact Mr. Yoakum by email at JYoakum@sjcehd.com or by telephone at (209) 468-3147. <br /> This EHD approval is limited to the methods proposed for the physical destruction of the above <br /> mentioned wells and management of investigation derived waste.
