Main Street December 1, 2014 <br /> Stockton, CA 95212 <br /> • Collecting soil samples at 5-foot intervals, two soil samples from each boring will be <br /> submitted for analysis. Soil samples will be submitted to a California State-certified <br /> laboratory and analyzed for TPH-g by EPA Method 8015 and BTEX and fuel <br /> oxygenates by EPA Method 8260B. <br /> • Development of installed groundwater monitoring wells, by purging up to a maximum <br /> of 10 casing volumes of water from each well or until the extracted groundwater is <br /> clear. A well head survey will be conducted by a licensed surveyor for all newly- <br /> installed groundwater wells. <br /> • Groundwater samples will be initially collected from the newly-installed groundwater <br /> monitoring wells and analyzed for TPH-g by EPA Method 8015 and BTEX and fuel <br /> oxygenates by EPA Method 82608. Once groundwater monitor wells are initially <br /> sampled, groundwater monitoring wells will continue to be sampled semi-annually <br /> during regularly scheduled monitoring events. <br /> • Disposal of soil and water generated from well installation and development activities <br /> temporarily stored in DOT-approved 55-gallon drums and/or a roll off bin. A licensed <br /> disposal contractor will transport the drums to an appropriate disposal or treatment <br /> facility. <br /> The EHD tentatively approves the WP submitted by ATC, contingent upon successful <br /> completion of the following EHD requirements: <br /> • All newly-installed groundwater monitoring wells must be sampled and analyzed for <br /> constituents of concern quarterly for the first year. Once the quarterly analysis for <br /> groundwater monitoring wells have been submitted to EHD and reviewed, continue <br /> to sample the wells semi-annually until further notice from EHD. <br /> • Notify EHD a minimum of five working days for all field work activities. <br /> • Submit a completed San Joaquin County Well and Boring Permit Application form <br /> that describes the construction of the wells as outlined in the work plan along with <br /> a site map showing the locations of all wells approved to be constructed. .A <br /> completed and signed Master File Record form that indicates who the billing party <br /> will be. In addition, a copy of an encroachment permit issued by the City of <br /> Stockton and/or an access agreement with private property owners for all off-site <br /> borings and groundwater monitoring wells locations. A permit fee for CPT Borings <br /> per parcel is $130, along with a $390 Inspection Activity fee per parcel. Once the <br /> permit has been issued, the EHD requires at least five working days notification for <br /> scheduling the inspection appointments. <br /> • All CPT soil borings and groundwater sample borings are required to be grouted <br /> from the total depth to surface grade with neat cement. <br /> • All investigation derived waste (IDW); including soil cuttings, groundwater, and <br /> water generated from well destruction activities, shall be properly containerized, <br /> labeled, stored and disposed. A signed manifest from the receiving facility for the <br /> disposal of the IDW must be submitted to EHD. <br />
