r n <br /> THIRD QUARTER,2001 <br /> GROUNDWATER MONITORING REPORT <br /> ,JAMAR OIL DISTRIBUTION <br /> SEPTEMBER 2001 <br /> ' John P Cummings &Associates JPCA conducted a subsurface investigation on September 8-9, <br /> g (JPCA) <br /> 1999, by installing and sampling three monitoring wells (MW-1, MW-2, and MW-3) MW-1, <br /> ' MW-2, and MW-3 were drilled to approximately 78 feet bgs, 74 feet bgs, and 74 feet bgs, <br /> respectively and sampled every 5 feet Groundwater samples were collected from the three wells <br /> on September 23, 1999 <br /> 1 <br /> On August 21, 2000, JPCA did a further subsurface investigation by drilling and sampling one soil <br /> ' boring (B1) and installing and sampling two additional monitoring wells (MW-4 and MW-5) <br /> The soil boring was drilled to approximately 115 feet bgs and sampled every 5 feet MW-4 and <br /> MW-5 were drilled to approximately 75 feet bgs with soil samples collected every 5 feet <br /> ' Groundwater samples were collected from MW-1,MW-2, MW-3, MW4 and MW-5 on August <br /> 30, 2000 The results of this investigation are documented in JPCA's 2"d Phase H Environmental <br /> Assessment Report dated October 25, 2000 <br /> After reviewing the October 25, 2000 report, the San Joaquin County Public Health Services, <br /> Environmental Health Division(County) determined the report to be incomplete according to <br /> specifications set forth in Appendix A, Tn-Regional Board Staff Recommendations for <br /> lie Preliminary Investigation and Evaluation of Underground Tank Sites(Appendix A) The County <br /> requested that additional information would be necessary to complete the report <br /> In a letter dated December 5, 2000,Ms Carol Oz with the County responded to the October 25, <br /> 2000 report with the following directives <br /> ® Further investigation of soil and groundwater is necessary to determine vertical <br /> ' and lateral extent of hydrocarbons in soil and groundwater <br /> ' 0 Provide information lacking from previous reports of findings for this site <br /> a Develop a workplan to include a proposal for contaminated soil and groundwater <br /> remediation <br /> e Initiate quarterly groundwater monitoring and continue until otherwise directed by <br /> ' the County, <br /> ® Submit quarterly monitoring reports within 60 days of the quarterly monitoring <br /> ' Reports must be in accordance with Appendix A <br /> Asa result ofthese directives,JPCA submitted a Remediation Proposal dated January 31,2001 The <br /> ' County responded to this proposal in a March 8, 2001 letter to Mr Jay Mcllrath of JAMAR <br /> ' 2 <br />
