The District requests that your firm submit a technical proposal to perform the project. Your proposal must be prepared in accordance with the following attachments: Attachment 1 - List of Consultants Receiving Request for Proposal (RFP) Attachment 2 - Project Background, Activities, and Schedule Attachment 3 - Required Proposal Contents and Format Attachment 4 - Scope of Services Attachment 5 - Interview Format, and Proposal and Interview Evaluation Criteria Attachment 6 - MBE/EEO/AA Guidelines Professional Services Package Attachment 7 - Sample Contract Attachment 8 - Available soil and groundwater data, and site location maps In order to effectively evaluate your proposal, all questions in the RFP should be addressed in the order listed. Please note that material submitted in addition to that specifically required in the attachments will not be reviewed. The proposal should stand on its own rather than refer to material provided in the Statement of Qualifications. To be considered, five (5) copies of your proposal must be received at P.O.
