EHD MW installation permit applications and fees must be submitted to EHD by October 1, <br /> 2008. <br /> 2. After the MW permits are issued by EHD, work must be performed and completed within 60 <br /> days. <br /> 3. A report of findings with conclusions and recommendations must be submitted to EHD by <br /> January 1, 2009. <br /> 4. When the additional site assessment data becomes available, the new information and <br /> previous data already collected will be submitted to EHD and CVRWQCB in the context of a <br /> Site Conceptual Model (SCM). <br /> 5.
