US Rentals <br /> Page 2 of 2 <br /> PHS/EHD requires finther investigation and documentation for the site as follows: <br /> 1) Soil borings with soil sampling at various locations at the site to determine the <br /> vertical and lateral extent of contamination is necessary. A monitor well should be <br /> installed closer to (ideally 10 to 15 ft. east) of the former underground tank location. <br /> This monitoring well should be screened from 50 to 70 feet below grade. Submit <br /> PHS/EHD permit application, work plan and permit fees to this department. Include <br /> as part of the work plan a cross section diagram which shows the contamination <br /> plume based on data obtained during investigation of this site to date. <br /> 2) Conduct a survey of domestic, municipal, agricultural and other wells within a 2000 <br /> foot radius of this site. Contact the Department of Water Resources, municipal water <br /> suppliers and other sources to identify these wells. Provide the location and <br /> construction of these wells in the work plan to be submitted to PHS/EHD. <br /> 3) Supply PHS/EHD with copies of manifests for all soils removed from site during, <br /> over-excavation activities <br /> 4) Provide an as-built plan of the existing soil vapor extraction framework. <br /> Submit the work plan to PHS/EHD by May 30, 1998. Continued quarterly <br /> sampling and reporting is required at the site. Notify PHS/EHD 48 hours prior to <br /> sampling events.
