GENERAL PROVISIONS: <br /> 1. Bidders must be properly licensed in accordance with the laws <br /> of the State of- California. <br /> 2. Liability and Property Damage: The contractor awarded this <br /> work .will be required to supply the City of Stockton, Risk <br /> Management Division, with a certificate of insurance and an <br /> additional insured endorsement (Form B) prior to starting <br /> subject work. <br /> Required Insurance Limits are as follows: <br /> A. Limit of Liability for injury or accidental death: <br /> One Person . . . . . . . . . . . $1, 000, 000 min. <br /> One Accident . . . . . . . . . . . . $1, 000, 000 min. <br /> B. Limit of Liability for property damage: <br /> One Accident • • <br /> . . . . $1, 000, 000 min. <br /> Aggregate LiabilityforLoss . . . . $1, 000, 000 min.' <br /> 3 . Contractor will provide Worker's Compensation Insurance for <br /> employees as required by the Labor Code. Said insurance shall <br /> be evidenced by an approved insurance certificate filed with <br /> the City Risk Management Division. <br /> 4 . Labor Standards Provisions: The following specifications <br /> shall apply to any public works contract, subcontract, or <br /> lower-tier subcontract exceeding $500.00: <br /> A. The contractor awarded this contract, and any <br /> subcontractor or lower-tier subcontractor, shall comply <br /> with the Labor Standards Provisions set forth in the <br /> California Labor Code and Administrative Code regarding <br /> the payment of prevailing wages on public works. <br /> B. Employees performing work on this public works project <br /> shall be paid the current prevailing rate of per diem <br /> wages pursuant to Section 1773 .2 of the Labor Code. The <br /> prevailing rate of per diem wages is meant to include: <br /> 1. The prevailing basic straight-time hourly rate of <br /> pay; <br /> 2. The prevailing rate of holiday and overtime pay; <br /> 3. The prevailing rate of employer payments for <br /> specified fringe benefits; <br /> 4. Travel time and subsistence pay. <br /> C. A copy of the prevailing wage rate determination shall be <br /> made available to the prime contractor and shall be <br /> posted at the project site or home office. Additional <br /> copies of the determination may be obtained from the City <br /> Clerk's office. <br /> REV. 10/91 <br />
