Based on the field observations made by my staff (and as we <br /> have experienced in the past) the neat cement grout was not successfully and confidently pumped down to full <br /> depth and up to surface grade of SP-4A. It was gravity fed, after attempts to pump the grout down the well <br /> failed. Additionally, the grout mixture used to seal this well did not meet the standards for an approved seal <br /> mixture,as additional water was used to dilute the mixture. <br /> We are requiring wells 5P-18,5P-3A,5P-5A,SP-5B, SP-5C,5P-6A, 5P-6B, SP-6C AND 5P-4A be removed, by over <br /> drilling with an augur two times the original borehole diameter. <br /> 1 <br />
