The top of well U-9 is protected by a vault box, <br /> . locking well cap, and lock Well construction details are presented with the Log of Boring <br /> in Appendix B. <br /> Well U-9 was installed with 30 feet of screen, as proposed in GSI's Work Plan SJCEHD <br /> Permit #13207 was issued with the condition that the well screen interval be limited to 20 <br /> feet On August 25, 1997, SJCPHS issued a letter stating that GSI and Unocal had failed <br /> to follow permitting requirements GSI submitted a Ietter to SJCPHS dated September 4, <br /> 1997, which responded to the SJCPHS letter To date, GSI has not received a response <br /> from SJCPHS <br /> Well Development, Monitoring, and Sampling <br /> Groundwater monitoring well U-9 was developed, monitored, and sampled by Gettler-Ryan <br /> Inc on August 25, 1997 Depth to groundwater was measured and the wells checked for <br /> the presence of floating product prior to purging and sampling Floating product was not <br /> present in well U-9 Well development field data sheets are presented in Appendix C <br /> MPDS will incorporate the newly installed groundwater monitoring well into the quarterly <br /> monitoring and sampling program beginning with the fourth quarter 1997 <br /> 482721-2 4 <br />
