• Well Installation Report - Former Unocal Service Station No 0187 <br /> October 21, 1997 <br /> soil, collected soil samples for possible chemical analyses, and prepared a log of the <br /> boring GSI Field Methods and Procedures are presented in Appendix A The Log of <br /> boring U-9 is presented in Appendix B Location of well U-9 is shown on the Site Plan <br /> (Figure 2) <br /> Drill cuttings generated during drilling activities were stockpiled on site and placed on and <br /> covered with plastic sheeting pending disposal Four soil samples were collected from the <br /> stockpile and were submitted to Sequoia Analytical Laboratory for compositing into one <br /> sample <br /> Well Installation <br /> Groundwater monitoring well U-9 was constructed using 2-inch-diameter Schedule 40 <br /> polyvinyl chloride (PVC) casing with 30 feet of 0.02-inch machine-slatted screen The <br /> screen in well U-9 extends from 25 to 55 feet bgs The annular space around the well <br /> screen was packed with Lonestar #3 graded sand The sandpack was followed by a <br /> bentonite transition seal and then neat cement containing approximately 5% bentonite <br /> powder to approximately 1 foot bgs. The top of well U-9 is protected by a vault box, <br /> . locking well cap, and lock Well construction details are presented with the Log of Boring <br /> in Appendix B. <br /> Well U-9 was installed with 30 feet of screen, as proposed in GSI's Work Plan SJCEHD <br /> Permit #13207 was issued with the condition that the well screen interval be limited to 20 <br /> feet On August 25, 1997, SJCPHS issued a letter stating that GSI and Unocal had failed <br /> to follow permitting requirements GSI submitted a Ietter to SJCPHS dated September 4, <br /> 1997, which responded to the SJCPHS letter To date, GSI has not received a response <br /> from SJCPHS <br /> Well Development, Monitoring, and Sampling <br /> Groundwater monitoring well U-9 was developed, monitored, and sampled by Gettler-Ryan <br /> Inc on August 25, 1997 Depth to groundwater was measured and the wells checked for <br /> the presence of floating product prior to purging and sampling Floating product was not <br /> present in well U-9 Well development field data sheets are presented in Appendix C <br /> MPDS will incorporate the newly installed groundwater monitoring well into the quarterly <br /> monitoring and sampling program beginning with the fourth quarter 1997 <br /> 482721-2 4 <br />
