Violation: Item # 301 CCR 66262.34(d)(2) <br />Facility not maintained to minimize the release of a hazardous waste. <br />a. HealthCare Clinical Laboratory — Hazardous Waste staining mixture poured off into containers in two <br />sinks. Both sinks showed stains from drips, splashes, over pours, etc. <br />b. In each of the two sinks there is an unplugged drain allowing the release of hazardous waste caused by <br />drops, splashes, and over pours. <br />Correction: Item a) was corrected on site during the inspection. Plastic tubs were placed into each of the sinks <br />to capture and prevent hazardous waste from entering the sink drains, caused by Drips, splashes and over pours. <br />b) Each of the two sinks drains have been sealed off to prevent residue from any drips, splashes, or over pours <br />from being released from the sink drains. Each of the two sinks have been deemed dedicated small quantity <br />hazardous waste areas for the use of pouring off small quantity Hazardous waste into satellite containers. <br />Violation: Item # 403 CCR 66262.34(d)(2) <br />Failed to keep hazardous waste containers closed except when adding or removing hazardous waste. <br />c. One 55g drum of viral disinfectant <br />d. One 55g drum of methanol, acetone, were seen open. <br />Correction: Containers closed on April 21, 2017. Discussed with the staff the importance of keeping these lids <br />closed except when adding or removing hazardous waste. <br />Violation: Item # 603 CCR 66262.34(e)(1)(C) <br />Failed to mark initial accumulation start date on Satellite containers. <br />a. One 1 g container in main lab did not have an initial start accumulation date <br />b. One 1 g container in main lab had an incomplete initial start accumulation date <br />c. Several 1 g containers in main hazardous waste storage area had dates marked in pen on plastic that <br />wiped off or was illegible <br />d. Several 1 g containers in main hazardous waste storage area did not have an initial start accumulation <br />date <br />e. Several 1 g containers had incomplete initial start accumulation dates (date, month w/o year) <br />Correction: (a -e) Clinical Laboratory staff educated on the requirement of placing start accumulation dates on <br />all Hazardous Waste labels to include day, month and year. Start accumulation dates to be documented on <br />Hazardous Waste labels with permanent ink sharpie, to prevent wiping off or smearing and eliminating the <br />opportunity of illegible date.
