Violation: Item # 605 CCR 66262.34(f) <br />Failed to completely label containers or portable tanks of hazardous Waste at time of inspection. <br />a. Printed labels are being used for the hazardous waste being generated in three labs. <br />b. The labels have multiple generator addresses and multiple compositions (contents) preprinted on them. <br />c. The labels have all 4 hazardous properties, toxic, reactive, Ignitable, and corrosive marked on them. <br />d. Two 1 g containers with original "Methyl Alcohol" label only had handwritten word "waste" and did not <br />have a hazardous waste label with any of the required information. <br />Correction: (a — d) Individual labels have been created for the two different locations. Two labels are specific <br />for the Clinical Laboratory located at 1800 N. California St. Stockton, CA 95204. (see attachment #3 and 4) Five <br />hazardous waste labels have been created for use at the HealthCare Clinical Laboratory 2102 N. California St., <br />Stockton CA 95204 location. (see attachment 5-9). The third laboratory site Stockton Pathology Medical Group, <br />has contracted with In genium, effective May 26, 2017 to be their Regulated Waste management provider. <br />d. Laboratory staff handling hazardous waste have been educated on hazardous waste label requirements and <br />the mandatory use of the labels on hazardous waste containers. (See attachment #1 and #2) <br />Violation: Item # 3030 HSC 25189.5(d) <br />Any person who knowingly treats or stores any hazardous waste at a facility which does not have a permit from <br />the department. <br />a. Waste was being transported from Stockton Pathology to SJMC. <br />Correction: On April 22, 2017 John Kendle, Senior Service Area Director, contacted the Partners of Stockton <br />Pathology Group at 2131 North California Street, Dr. Stephen Connolly & Dr. David Jensen and informed them <br />that effective immediately St. Joseph's Medical Center would no longer be able to transport, store or dispose of <br />Hazardous Waste Materials for Stockton Pathology Group. <br />b. Stockton Pathology Group has since contracted with Ingeium for Hazardous Waste Disposal that can be <br />viewed upon request. <br />
