• <br /> 2.0 SCOPE OF WORK <br /> The objective of this investigation was to evaluate the lateral and vertical extent of any impacted <br /> sod and/or groundwater associated with the former USTs To achieve the project objective, sod <br /> and groundwater-grab samples were collected for laboratory analysis from five soil borings Due <br /> ' to the depth and distribution of petroleum hydrocarbons in soil and groundwater beneath the <br /> site, the approved scope of work was modified in the field with approval from PHSIEHD The <br /> scope of work was modified to drill five soil borings instead of 11, to a depth of up to 43 feet <br /> instead of 25 feet <br /> 2.1 Protect Planning <br /> The project planning task included the acquisition of required permits, routine notifications and <br /> underground utility surveys, and the preparation of a health and safety plan <br /> ' 2 1 1 Permittinq and Utility Clearance <br /> To perform the proposed scope of work, two permits were obtained from PHSIEHD (Appendix <br /> A) Permits were obtained for the drilling operations at the subject site and the 99 Recycling <br /> Center (430 South Moffat Boulevard), immediately east of the site Permission to access the 99 <br /> ' Recycling Center property was granted by Frank & Audrey DeRose (property owners) on June <br /> 16, 1999 (Appendix A) Since field work was not conducted in South Moffat Boulevard, an <br /> encroachment permit was not required <br /> At least two working days prior to any field work, PHSIEHD was notified so that regulatory <br /> inspections could be scheduled On July 2, 1999, prior to the first phase of field work, <br /> Underground Service Alert (USA) was notified to mark any underground utilities (USA Ticket <br /> #515628) On August 16, 1999 prior to the second phase of field work, USA was re-notified <br /> r (USA Ticket#559389) <br /> 1 2 1 2 Health and Safety Plan <br /> Prior to initiating fieldwork, a site-specific health and safety plan was prepared in accordance <br /> with 29 CFR 1910 120 The health and safety plan includes safety procedures for work to be <br /> performed, chemical hazard information, site safety officers, and a medical emergency location <br /> A copy of the health and safety plan is included as Appendix B <br /> ' 7 <br />
