TASK-BY-TASK RISK ASSESSMENT <br /> (Complete One Sheat for Each Task► <br /> TASK DESCRIPTION <br /> Task 1 — Phase I Environmental Assessment— Due Diligence per ASTM Standard Practice for Environmental Site Assessments. <br /> Project activities will consist of conduct of facility walkthrough(nspection, interviews, records review, local agency records <br /> review and contacts. <br /> EQUIPMENT REQUIRED/USED <br /> (Be specific,e.g.,hand tools,heavy equipment. inatrumants, PPEI <br /> ASTM Checklist, site specific PPE e.g., hard hate or bump hats, safety glasses, ear protection (per client safety procedures). <br /> Other potential tools include hand tools e.g., screwdriver, flashlight, knife for limited use during facility inspection. <br /> POTENTIAL HAZARDSIRISKS <br /> CHEMICAL <br /> ® Hazard Present Risk Level: ❑ H ❑ M ® L <br /> What Justifies Risk Level? No intrusive activities to be conducted. Only walkthrough of facility to evaluate potential activities <br /> which could cause an impact to the environment. Actual hazard depends on the type of facility and operations conducted. <br /> PHYSICAL <br /> ® Hazard Present Risk Level: ❑ H ❑ M ® L <br /> What Justifies Risk Level? Slip, trip, and fall hazards during conduct of site walk. Overhead bump hazards. personnel will wear <br /> appropriate PPE per client procedures. Personnel will be cognizant of surrounds and potential hazards. <br /> BIOLOGICAL <br /> IN Hazard Present Risk Level: ❑ H ❑ M ® L <br /> What Justifies Risk Level? Potential. Depends on the facility and level of operations. More operations less hazards, operations <br /> or vacant more hazards from weeds,plants, rodents, snakes.
