Information and instructions for obtaining a permanent California EPA ID number (form <br /> enclosed): Complete the application for a California Hazardous Waste Permanent ID <br /> Number and submit it to the DTSC by fax, mail, or email. <br /> 12. Board of Equalization UST Storage Fee Account Number— Call (916) 322-9669 to <br /> obtain a current number for your facility. <br /> 13. San Joaquin Valley Air Pollution Control District—Call (209) 557-6400 to obtain an <br /> operating permit for the UST vapor recovery system. <br /> 14. Food facility operations —Call Leyna Huynh at (209) 468-3446 for operating permit <br /> information for opening the retail food facility. A consultation appointment may be <br /> needed. <br /> 15. Go to: http.//www.sicehd.com/Programs/Unitiii/unitiii.htm for additional information, <br /> updates, regulations and forms. The most current versions of all forms must be <br /> submitted. <br /> Also included is a list of our upcoming training classes offered to San Joaquin County <br /> businesses for FREE. Please consider attending one of these free hazardous waste or <br /> underground storage tank classes. <br /> The following is the required follow up and routine testing for your facility: <br /> 1. Annual monitoring system certification, leak detector testing, and spill container testing <br /> were last performed on May 16, 2016. These tests are required once every 12 months. <br /> 2. Secondary containment testing was conducted on May 11, 2016.
