EI Dorado St., Stockton, CA 95202 SR0072M <br /> The San Joaquin County Environmental Health Department (EHD) has been informed that as of <br /> September 21, 2016, you are the new owner or operator of the underground storage tanks <br /> (USTs) at the above mentioned facility. Please complete the following items within 30 days of <br /> the close of escrow [CA Health and Safety Code section 25284(b) & (c)]: <br /> 1. Create an account and obtain facility access to the facility on the California <br /> Environmental Reporting System (CERS) by going to httpJ/cers.calepa.ca.gov. Ensure <br /> that all required uploads are completed in a timely manner. <br /> 2. Underground Storage Tanks — Facility Form — Provide all current information. Log into <br /> CERS, update the required information, and submit for review by the EHD. (Note: Any <br /> change of information must be updated in CERS within 30 days of the changes.) <br /> 3. Underground Storage Tanks—Tank Forms — Provide all current information in CERS. <br /> Complete a separate tank form for each tank. <br /> 4. Certification of Financial Responsibility/Letter from Chief Financial Officer: Complete the <br /> forms provided, and upload in CERS. This document is required to be submitted <br /> annually. <br /> 5. Underground Storage Tank Monitoring and Response Plans: Upload in CERS. Retain an <br /> approved copy of these plans on site for future reference. <br /> 6. If you are the owner, you must submit a notification form identifying the "Designated UST <br /> Operator" for each facility owned. A notification form is enclosed for your use. <br /> 7. Report#5021: Update all information for all programs at this site including the UST and <br /> Hazardous Waste programs, sign and date. Bring with all other forms to our accounting <br /> office to pay for transfer fees. <br /> 8. Go to: http.//www.waterboards.ca.gov/cwphome/ust/regulatoN/index.html#rq to obtain <br /> the current statutes (Underground Storage of Hazardous Substances) and UST <br /> Regulations that you are required to have for your monitoring and response plan. <br /> 9. Enclosed for your information is a brochure of Best Management Practices (BMPs) for <br /> Retail Gasoline Outlets that you can use as part of your BMPs for your facility. <br /> 1 <br />
