The 87 spill container was replaced on 4/20/18. Notification was made to the EHD, but the technician name and certifications were not found on site. Like-for-like repairs relating to UST system monitoring and release detection may be performed by a properly licensed, trained, and certified contractor pursuant to the EHUS policy and a permit if all required paperwork is placed on site. All other repairs must have a permit submitted to and approved by the EHD prior to beginning work. Immediately provide the required paperwork to store on site for this repair and ensure all required paperwork is maintained on site in the future. This is a minor violation.
