Please complete and return this form. <br /> -Documentation of the work(i.e.work order, report by the technician)by the technician who'd trouble-shoot the diesel sump and <br /> concluded there was no leak and no repair required as you'd stated in the Return to Complaince(RTC)statement.(#22/43) <br /> You stated you have informed your Designated Operator(DO)Affordatest that they need to include all alarms on the DO Report; <br /> this is fine.
