(Item #22) I've included the blank Unauthorized Release form as <br /> attachment. Please complete and return this form. <br /> - Documentation of the work(i.e. work order, report by the technician) by the technician who'd trouble-shoot the diesel sump and <br /> concluded there was no leak and no repair required as you'd stated in the Return to Complaince (RTC) statement. (#22/43) <br /> You stated you have informed your Designated Operator(DO)Affordatest that they need to include all alarms on the DO Report; <br /> this is fine. Since you have switch DO to Robert Barnhart, you may want to remain him of the same. <br /> I have also included the"approved" Monitoring Plan and Response Plan signed off by me as attachments.
