(Apex)has been authorized by Williams Tank Lines, Inc. (Williams) to
submit this application for soil borings to evaluate the lateral and vertical extent of petroleum-
impacted soil at the subject site and to identify contaminate transport and exposure pathways in
order to assess the risk to the environment and human health posed by the site. All work is being
done based on the release of gasoline from a tanker truck crash that occurred on August 31, 2013.
Apex proposes to advance three direct push soil borings (SB-1 through S13-3) at the subject site
to approximately 10 feet below ground surface. The three direct push borings will be used to
evaluate the lateral and vertical extent of petroleum hydrocarbon impacted soil and groundwater.
The location of the site and the proposed direct push borings are shown on Figures 1 and 2.
Apex personnel may modify the total depth of the borings as site conditions warrant.
Soil samples will be collected continuously for logging purposes and up to three soil samples
will be collected from the direct push soil borings.
Upon completion of the borings and sampling activities, the borings will be grouted to the
surface per SJCEHD guidelines.
Included with this letter please find the following:
• One Master File Record(MFR);
• Figures 1 (Site Vicinity Map) &Figure 2 (Proposed Boring Location Map);
Please do not hesitate to contact me if you have any questions or require additional information.
I may be reached at 916-851-0174.
Sincerely,
APEX ENVIRO CH, I
Michael van den Enden
Project Geologist
Cc: File
