(Apex)has been authorized by Williams Tank Lines, Inc. (Williams) to <br /> submit this application for soil borings to evaluate the lateral and vertical extent of petroleum- <br /> impacted soil at the subject site and to identify contaminate transport and exposure pathways in <br /> order to assess the risk to the environment and human health posed by the site. All work is being <br /> done based on the release of gasoline from a tanker truck crash that occurred on August 31, 2013. <br /> Apex proposes to advance three direct push soil borings (SB-1 through S13-3) at the subject site <br /> to approximately 10 feet below ground surface. The three direct push borings will be used to <br /> evaluate the lateral and vertical extent of petroleum hydrocarbon impacted soil and groundwater. <br /> The location of the site and the proposed direct push borings are shown on Figures 1 and 2. <br /> Apex personnel may modify the total depth of the borings as site conditions warrant. <br /> Soil samples will be collected continuously for logging purposes and up to three soil samples <br /> will be collected from the direct push soil borings. <br /> Upon completion of the borings and sampling activities, the borings will be grouted to the <br /> surface per SJCEHD guidelines. <br /> Included with this letter please find the following: <br /> • One Master File Record(MFR); <br /> • Figures 1 (Site Vicinity Map) &Figure 2 (Proposed Boring Location Map); <br /> Please do not hesitate to contact me if you have any questions or require additional information. <br /> I may be reached at 916-851-0174. <br /> Sincerely, <br /> APEX ENVIRO CH, I <br /> Michael van den Enden <br /> Project Geologist <br /> Cc: File <br />
