Provide a copy of the Fire Department (District) Permit to Public Health <br /> Services/Environmental Health Division (PHS/EHD) . <br /> 2. Provide a plot plan showing the name of property owner, facility <br /> address, parcel number, tank size, product stored, method of abandonment <br /> and tank(s) distance to all wells within 1,000 feet of the abandoned <br /> tank(s) . <br /> Alternative #2 <br /> 1. Provide a record of materials purchased to abandon tank(s) . <br /> 2. Provide a plot plan showing the name of property owner, facility <br /> address, parcel number, tank size, product stored, method of abandonment <br /> and tank(s) distance to all wells within 1,000 feet of the abandoned <br /> tank(s) . <br /> Alternative #3 <br /> 1. Provide a signed statement from the property owner stating date tank <br /> abandoned. <br /> 2. Provide a plot plan showing the name.of property owner, facility <br /> address, parcel number, tank size, product stored, method of abandonment <br /> and tank(s) distance to all wells with 1,000 feet of the abandoned <br /> tank(s) . <br /> All underground storage tanks abandoned after July 1, 1984 shall follow the <br /> procedures set by State Law.
