If loud noise is present or normal conversation becomes <br /> ' difficult, hearing protection in the form of ear plugs, or equivalent, will be required. <br /> 5.2.4 Electric Shock <br /> All electrical equipment to be used during field activities will be suitably grounded and <br /> insulated. Ground fault circuit interrupters will be utilized with all heavy electrical <br /> ' equipment to reduce the potential for electrical shock. <br /> ' 5.2.5 Excavations <br /> Trenching for the installation of remedial systems may occur during the course of the <br /> ' project. The trenching activities for the installation of conveyance piping are expected <br /> to be to a depth no greater than 3 feet below ground surface. If more extensive <br /> excavation activities are necessary, the following requirements will be observed. <br /> ' Workers shall not enter excavations or trenches greater than 4 feet in depth without <br /> first obtaining a permit for confined space entry from the Director of Health and <br /> ' Safety. The walls and spaces of all excavations more than 4 feet deep that will be <br /> entered by personnel shall be guarded by shoring, sloping, or benching. Air <br /> monitoring will be performed in excavations greater than 4 feet in depth into which a <br /> person will descend. Air monitoring will include the use of an LEL/02 meter and <br /> photoionization detector/flame ionization detector (PID/FID), as appropriate. Side <br /> slopes shall not be steeper than 1:1 without a written report from a qualified civil or <br /> geotechnical engineer. All excavations shall be in accordance with the Cal/OSHA <br /> Excavation Safety Standard, 8 CCR 1541. <br /> The SSO shall inspect excavations daily. If there is evidence that a cave-in or slide is <br /> possible, all work shall cease until the necessary safeguards have been taken. <br /> Excavated material shall be placed far enough from the edge of the excavation (a <br /> ' minimum of 2 feet) so that it does not fall back into the opening. At the end of each <br /> day's activities, all open excavations will be clearly marked and secured to prevent <br /> nearby workers or unauthorized personnel from entering them. <br /> ' 5.2.6 Underground and Overhead Utilities <br /> ' The location of all underground pipes, electrical conductors, fuel lines, and water and <br /> sewer lines must be determined before soil intrusive work is performed. All lines must <br /> be de-energized, blocked out, or blinded where feasible.
