Levine-Fricke-Recon • ,
Section:5
Date Initiated:September 23, 1996 ,
Revision No.: 1
Date Revised:August 21, 1998
5.2.7 General Safe Work Practices
All personnel, including LFR and subcontractor personnel, shall bring to the attention '
of the SSO any unsafe condition or practice associated with site activities. LPL will
also be notified, as appropriate, of any injuries occurring at the Site. ,
Hands, face, and all other potentially contaminated areas shall be thoroughly
cleaned before smoking, eating, or leaving the Site. ,
• Respiratory devices may not be worn with beards, long sideburns, or under other
conditions that prevent a proper seal.
• All accidents and/or injuries shall be immediately reported to the SSO. If ,
necessary, a first report will be initiated by the SSO.
• Periodic safety briefings will be held to discuss current site conditions, field tasks ,
being performed, planned modifications, and work concerns.
• Workers shall maintain good housekeeping practices during field activities in order '
to maintain a safe working environment. The work site shall be kept free of debris,
waste, and trash at all times.
• The "buddy system" shall be used whenever appropriate. '
Workers shall use extreme caution when conducting activities near open manholes.
Before opening manholes, a perimeter will be established (using traffic delineators and '
caution tape, or other equally effective means) at a minimum of 5 feet in all directions
to restrict access to the area. Only authorized persons will be allowed within the
perimeter, and equipment shall be replaced whenever there is a break in the work. '
Under no circumstance will an opening to underground structures be left unattended.
5.2.8 Other Physical Hazards '
Vehicular traffic presents opportunities for serious injury to person or property. Heavy
traffic on Douglas Road, Pacific Avenue, and Benjamin Holt Drive may endanger ,
persons entering and leaving the shopping center by vehicle through any one of the
access drives. Pedestrians crossing or walking along the highways are in danger.
Within the property, the majority of traffic consists of motor vehicles belonging to, and ,
operated by, shopping center visitors and employees. Trucks and vans making
deliveries or picking up shipments account for much of the rest of the traffic. The
various lanes, parking spaces, and turning movements present points of potential ,
conflict. Workers and other pedestrians are clearly at risk during periods of heavy
traffic and during delivery when vans and trucks are maneuvering the lanes and
backing up to loading docks. Risk from all motor vehicle operations is minimized by ,
good operating practices and alertness and care on the part of workers and pedestrians;
Page F-14 RIFSWP-AppF-ReV1-04111:SRL '
